All required fields, indicated by highlights, must be completed, and a valid tax exemption certificate must be attached. If a section is not applicable, please mark it as "N/A." You will receive a confirmation once your tax-exempt status has been verified.

Important: If you place an order before your tax exemption is approved, taxes will be charged. Unfortunately, we are unable to refund taxes on orders placed prior to approval.
